Description

The capacity to feel pain serves a protective function: It elicits coordinated reflex and behavioral responses to minimize tissue damage, whether it is imminent or already occurring. A series of excitability changes in the peripheral and central nervous systems lead to severe but reversible pain hypersensitivity in the inflamed and surrounding tissue in the event that tissue damage cannot be avoided. Because any contact with the damaged part is avoided until healing has occurred, this procedure aids in wound repair. Contrarily, persistent pain syndromes result in suffering and distress without any biological advantage. Such maladaptive pain is referred to as neuropathic pain and typically results from damage to the nervous system (the peripheral nerve, the dorsal root ganglion or dorsal root, or the central nervous system). These syndromes are made up of a complicated combination of positive symptoms like dysaethesia, paraesthesia and pain and negative symptoms like sensory deficits like partial or complete loss of sensation.

Pharmacotherapy for neuropathic pain has been disappointing, with the exception of trigeminal neuralgia, which responds well to cortisone. Non-steroidal anti-inflammatory medications and opiates frequently cause resistance or insensitivity in neuropathic pain patients. Tricyclic or serotonin and norepinephrine uptake inhibitors, antidepressants, and anticonvulsants, all of which have limited efficacy and undesirable side effects, are typically used empirically to treat patients. Although transcutaneous nerve stimulation may provide some relief, neurosurgical lesions play a negligible role and functional neurosurgery, such as dorsal column or brain stimulation, is contentious.

The effects of local anesthetic blocks that target trigger points, peripheral nerves, the plexus, dorsal roots, and the sympathetic nervous system are beneficial but temporary; longer-lasting blocks administered by phenol injection or cryotherapy have not been tested in placebo-controlled trials due to the possibility of irreversible functional impairment. Drugs like clonidine, steroids, opioids, or midazolam that are given continuously under the epidural is invasive, has side effects, and has not been adequately evaluated for their efficacy.

Ectopic nerve Activity

The aetiology of the insult to the nervous system or the anatomical distribution of the pain is currently used to classify neuropathic pain. Although this classification provides no framework for the clinical management of pain, it is useful for the differential diagnosis of neuropathy and, if available, for disease-modifying treatment. In this condition, the relationship between the aetiology, mechanisms and symptoms is complicated. It's possible that the same mechanisms are at work in the pain that comes from a variety of diseases. There is no unavoidable pain mechanism associated with a particular disease process; There are only a few affected patients, and there are no predictors for which patients will experience neuropathic pain. Multiple symptoms could be caused by a single mechanism. In addition, two patients may experience the same symptom in different ways. Lastly, a patient may have multiple mechanisms operating simultaneously, and these mechanisms may alter over time. Therefore, it is impossible to anticipate targets for the sensory neuron cell bodies that regulate neuropeptide transmitter concentrations in neuropathic pain patients.

NeuPSIG recently defined neuropathic pain as "pain arising as a direct consequence of a lesion or disease affecting the somatosensory system," according to the definition. This suggests that a lesion that affects either the central or the peripheral nervous systems can cause neuropathic pain. "pain initiated or caused by a primary lesion or dysfunction of the nervous system," according to the current IASP definition, In order to distinguish neuropathic pain from pain caused by neuroplastic plastic changes in response to strong nociceptive stimulation, the new definition proposed by NeuPSIG substitutes "disease" for "dysfunction. "To distinguish neuropathic pain from pain caused by lesions in other parts of the nervous system, such as pain caused by muscular spasticity caused by lesions of central motor pathways, the term "nervous system" is changed to "somatosensory system". For both clinical and research purposes, a proposed method for diagnosing neuropathic pain is presented.

Our approach is to treat patients with pain, or neuropathic pain, rather than neuropathy because somatosensory system diseases and lesions can be painful or not. The difficulty lies in distinguishing neuropathic pain from other types of pain and diagnosing the disease or lesion that is causing the pain. The current work did not fall under the scope of recommendations for traditional neurological diagnostic tests for the crucial step of confirming a somatosensory system lesion or disease. Although standard neurophysiological responses to electrical stimuli, like nerve conduction studies and somatosensory evoked potentials, are useful for demonstrating, locating, and quantifying damage along the central and peripheral pathways, they do not evaluate the function of the nociceptive pathways. We refer to recent guidelines for making the diagnosis of neuropathy; this review does not cover the diagnostic algorithms for neuropathic pain-causing central nervous system diseases and peripheral neuropathies.

Aetiology of Neuropathic Pain

The following are the goals of this article:1) determine the population's incidence and prevalence of neuropathic-type pain; 2) determine the sensitivity of the various methods for assessing patients with neuropathic pain; 3) determine the methods for evaluating standard treatments; and 4) propose, if necessary, new studies that may assist in elucidating ambiguous issues.

Ectopic impulse generation within the nociceptive pathways is what causes paroxysmal shooting pain and ongoing spontaneous pain in the absence of any external stimulus. Miconeurography has shown such spontaneous ectopic activity in neuroma afferent fibers in patients with stump and phantom pain as well as painful diabetic neuropathy. The high thresholds of nociceptors for mechanical, thermal, and chemical stimuli are reflected in the activation of unmyelinated (C-fibre) and thinly myelinated (A-fibre) nociceptive afferent fibers under physiological conditions, indicating potential tissue damage. When a person experiences neuropathic pain, these conditions drastically shift. Nociceptive afferents in the area, both those that have been injured and those that are not, exhibit spontaneous activity following a peripheral nerve injury. Increasing levels of voltage-gated sodium channel mRNA appear to correlate with ectopic activity, and increased sodium channel expression in intact and lesioned neurons may lower the action potential threshold until ectopic activity occurs. It is thought that central lesions cause similar changes in second-order nociceptive neurons, resulting in central neuropathic pain.
